Missed your Mounjaro or Zepbound dose? The 4-day rule, explained

· Based on FDA prescribing information & peer-reviewed sources · Updated 2026-10-31 · 5 min read

Quick answer

For Mounjaro and Zepbound (tirzepatide), the FDA label rule is the 4-day rule: if it has been 4 days (96 hours) or less since your missed dose, take it as soon as you can. If more than 4 days have passed, skip it and take your next dose on your usual day. Always keep at least 3 days (72 hours) between two doses, never take two doses to catch up, and confirm your situation with your prescriber.

What is the Mounjaro and Zepbound missed-dose rule?

For tirzepatide, sold as Mounjaro (type 2 diabetes) and Zepbound (weight management and sleep apnea), the rule is the 4-day rule. Per the FDA prescribing information:

  • 4 days (96 hours) or less since the missed dose: take it as soon as you remember.
  • More than 4 days since the missed dose: skip it, and take your next dose on your usual day.

Unlike the Wegovy rule, this one counts from the dose you missed, not from the next one coming up. So if your shot day is Sunday and you remember on Wednesday evening, you are still inside the window: take it. If you remember on Friday, you are past it: skip, and inject again on Sunday.

Why do people mix it up with the Wegovy rule?

Because both are weekly GLP-1 shots, and the numbers get swapped online all the time:

  • Mounjaro & Zepbound (tirzepatide): 4-day / 96-hour rule, counted from the missed dose.
  • Wegovy (semaglutide): 2-day / 48-hour rule, counted to your next dose. See the Wegovy rule.
  • Ozempic (semaglutide): take it within 5 days, otherwise skip.

If you have switched from semaglutide to tirzepatide, or the other way around, the rule you remember may belong to your old medication. Check the label of the pen or vial you actually use.

How do you change your shot day safely?

The tirzepatide label lets you move your weekly day, with one condition: at least 3 days (72 hours) between two doses. That also settles the most common follow-up question: after a late dose, when is the next one?

  • You took Sunday’s dose on Tuesday. Your next dose can stay on Sunday, because there are 5 days between them.
  • You took Sunday’s dose on Thursday. Sunday is only 3 days later, which is the minimum, so it is allowed. If you’d rather have more space, move your shot day to Thursday from now on.

The 72-hour rule is the line you never cross. Two tirzepatide doses closer than that is the “double dose” the label warns against.

Why does the “never double up” rule matter?

Because your last dose is still working. Tirzepatide’s half-life is about 5 days, so a large part of a dose is still active when the next one is due.

~5 days Tirzepatide's half-life on the US label. A week after a dose, close to 40% of it is still active. A second shot on top of it doesn't "catch you up". It stacks onto a level your body hasn't cleared, which is how people end up with their worst nausea and vomiting.

So one missed shot is not an emergency, and it is never fixed by adding more. You can see what a skipped week does to your level on the free tirzepatide half-life calculator: the curve dips, then rebuilds with your next regular dose.

Does the rule change for the KwikPen or vials?

No. The rule comes from the medicine, not the device. Zepbound is sold as single-dose pens, single-dose vials, 4-dose vials and a 4-dose KwikPen, and Mounjaro also comes as a KwikPen in many countries. The 4-day window and the 72-hour gap apply to all of them.

One practical point for multi-dose devices: a missed week does not mean a lost dose. The dose stays in the KwikPen or vial for your next injection, as long as you store it as the label says. Our guide to pen storage and travel covers temperatures.

When should you call your prescriber instead of just resuming?

Reach out before your next shot when:

  • You’ve missed two or more weekly doses in a row. After a long gap, side effects can feel like starting over, and your prescriber may restart you at a lower step.
  • You’re mid-titration and not sure which step to come back on.
  • You skipped a dose because of side effects. That is a dosing conversation, not a scheduling one.
  • You use birth control pills. The tirzepatide label advises a non-oral method, or adding a barrier method, for 4 weeks after starting and after each dose increase. That applies again if you restart and step up. See GLP-1s, birth control and pregnancy.
  • You accidentally took two doses close together. Call your prescriber or pharmacist, and get urgent help for severe vomiting or belly pain.

The honest fix: a reminder tied to the window

Most missed shots come from one thing: a busy week with no reminder tied to the actual 4-day window. A nudge the evening before and on shot day, plus a clear “you can still take it until Thursday” message, turns “did I miss it?” into a quick answer. FAQ

FAQ

What is the Mounjaro and Zepbound missed-dose rule?

Per the FDA label: take the missed dose as soon as possible if it has been 4 days (96 hours) or less since you missed it. If more than 4 days have passed, skip it and take the next dose on your regular day. Never take two doses within 3 days (72 hours) of each other.

Is the 4-day rule the same for Wegovy and Ozempic?

No. The 4-day rule belongs to tirzepatide (Mounjaro and Zepbound). Wegovy uses a 2-day rule based on how far away your next dose is, and Ozempic allows a late dose within 5 days. Use the rule on your own medication's label.

Can I change my Mounjaro or Zepbound injection day?

Yes. The label allows you to move your weekly day, as long as at least 3 days (72 hours) pass between two doses. Take your next dose on the new day and keep that day from then on.

Does the rule change for the KwikPen or vials?

No. The missed-dose rule comes from the medicine, not the device. It is the same for single-dose pens, single-dose vials, 4-dose vials and the 4-dose KwikPen.

What if I've missed Mounjaro or Zepbound for two weeks or more?

Talk to your prescriber before your next shot. After a long gap, side effects can feel like starting over, and your prescriber may restart you at a lower step instead of your last dose.
