Typical FDA-labeled schedule: 0.25 mg weekly for 4 weeks (starter, not effective for glycemic control), then 0.5 mg. If needed, increase after at least 4 weeks per step to 1 mg, then 2 mg max.

How GLP-1 titration works

Every GLP-1 starts low and steps up on a schedule — usually every 4 weeks. The starting dose isn't meant to do much; it lets your gut adapt so that later, effective doses are tolerable. Rushing this is the single most common cause of a miserable first month.

The standard ladders

These are the FDA-labeled escalation schedules the planner uses. Your prescriber may hold a step longer, or individualize the plan — especially for compounded or oral GLP-1.

  • Wegovy (semaglutide): 0.25 → 0.5 → 1 → 1.7 → 2.4 mg, stepping every 4 weeks.
  • Ozempic (semaglutide): 0.25 → 0.5 → 1 → 2 mg, at least 4 weeks per step.
  • Mounjaro / Zepbound (tirzepatide): 2.5 → 5 → 7.5 → 10 → 12.5 → 15 mg, 4 weeks per step.
  • Rybelsus (oral semaglutide): 3 mg daily for 30 days → 7 mg → 14 mg.

Why not just start high?

Side effects track how fast your level rises, not the absolute dose. Titration keeps that rise gentle. It's also why your medication level resets and rebuilds after each step — something GLPMate's free level plotter shows on your real schedule.

Titration questions

What is GLP-1 titration?

Titration is the gradual, stepwise increase of your dose over weeks — starting low and going up on a set schedule. It gives your body time to adjust, which greatly reduces nausea and other side effects. Every GLP-1 has its own labeled titration ladder.

How long does it take to reach the full dose?

Most schedules step up every 4 weeks. Wegovy reaches its 2.4 mg maintenance dose at about week 17; Mounjaro and Zepbound reach 15 mg over several months if needed. Your prescriber may hold you at a step longer if side effects need time to settle — that's normal.

Can I move up faster to lose weight quicker?

No — faster escalation mainly means worse side effects, not faster results, and prescribers won't do it. The dose that works is the lowest one that controls your appetite; many people do well below the maximum. Follow your titration schedule.

What does the calendar download include?

The .ics file adds an all-day reminder on each dose-change date — the days you step up to a new dose. Import it into Apple Calendar, Google Calendar or Outlook. For weekly shot-day reminders, GLPMate handles those in-app on your real schedule.

My prescriber gave me a different schedule — which is right?

Always follow your prescriber. This planner shows the standard FDA-labeled ladder, but compounded and individualized plans vary (slower steps, micro-dosing, holds). Use it to understand the shape, not to override your clinician.
