# GLP-1 side effects, week by week: what’s typical, what’s temporary, what’s a call-your-doctor

Quick answer

Most GLP-1 side effects are gastrointestinal, mild-to-moderate, cluster around dose increases, and fade within days to a few weeks as your body adapts. In the STEP trial, nausea affected about 44% of semaglutide 2.4 mg users but led only ~4.5% to stop treatment. Severe or persistent pain, repeated vomiting or dehydration are different — those need a clinician, not patience.

## The big picture first

GLP-1 side effects are mostly a story about your digestive system adapting to slower gastric emptying — and the adaptation is real. Trials and labels agree on the pattern: **gastrointestinal, early, dose-step-related, and usually transient**.

**44%** of participants on semaglutide 2.4 mg reported nausea in STEP 1 — yet only ~4.5% stopped for GI side effects. Common ≠ unbearable, and most of it passes.

## Weeks 1–2: the introduction period

On starter doses, many people feel little or nothing. When symptoms appear, it’s typically mild nausea or fatigue in the 24–72 hours after injecting — the window your level peaks. Small meals, slower eating and skipping heavy/fatty food on shot day are the standard first-line tactics.

## Weeks 3–6: dose steps and their echoes

Each titration step (weeks 4, 8, 12…) tends to bring a short symptom echo — a few rougher days while your body meets the higher level, then settling. Constipation often surfaces in this window as food volume drops; fluids, fiber and movement are the label-consistent basics. Reflux and burping (“sulfur burps” for a minority) also cluster here.

The practical skill in this phase is **separating step-echoes from trends**: a 6/10 nausea for two days after moving to 5 mg that fades to 2/10 is a normal echo. A 6/10 that stays 6/10 for two weeks is a conversation with your prescriber about pace.

## Weeks 6–12: the fade

For most people, GI symptoms genuinely decline as doses stabilize — the trials’ event curves drop after the escalation phase. Appetite suppression stays; the queasiness mostly doesn’t. If your chart shows the opposite shape (rising severity at a stable dose), that’s exactly the kind of pattern worth showing a clinician early.

## What helps, per the labels and trial protocols

- **Eat smaller, slower, earlier** — volume and speed drive most nausea.
- **Hydrate deliberately** — dehydration is the quiet escalation path (and the kidney-warning route on the labels).
- **Protein first, fiber daily** — protects muscle, manages constipation.
- **Time heavy meals away from your peak** (days 1–3 post-shot).
- **Ask about pacing** — holding a titration step longer is a labeled, normal move.

## The unambiguous “call your doctor now” list

- Severe, persistent abdominal pain — especially radiating to the back (pancreatitis warning)
- Repeated vomiting, inability to keep fluids down, or signs of dehydration
- Right-upper-abdomen pain, fever, or yellowing skin/eyes (gallbladder warning)
- Allergic-reaction signs: swelling of face/throat, trouble breathing
- Severe hypoglycemia if you also take insulin or a sulfonylurea

“Some nausea” gets you sympathy. “Nausea peaked at 6/10 for two days after each step, easing to 1–2/10 by day four — here’s the chart” gets you a _plan_: hold this step, adjust that habit, escalate on schedule.
